This workbook is written by culture change coach, activities professional and former state surveyor, Carmen Bowman owner of Edu-Catering: Catering Education for Compliance and Culture Change.
The workbook displays Carmen's depth of knowledge about the importance of getting to know those we serve, giving them the gift of being well known.
Get to know the whole person. Deep engagement occurs when homes get intentional about meeting the long-standing and good CMS requirement that "services are furnished to attain or maintain the resident's highest practicable physical, mental and psychosocial well being."
The workbook includes worksheets, resources, exercises and discussion topics, presenting both the philosophical and the practical.

This is a guide to help you really get to know a person. The Activity Interest Assessment is long, but don't let the length scare you! Aren't you tired of short assessments that typically only mimic the MDS and at the end we are still left wondering, "But, who is she?" Let's get beyond the institutional way of just filling out a form because we have to. This guide has been designed to help really get to know a new person coming to live in your midst (a new neighbor instead of a "new admit"). Isn't that what any of us would want - to be known? Really utilize this tool in short intervals to build rapport and relationships, that's what surveyors do with their many resident interview questions.
Interest prompts are included to help you find out more. Instead of just having a prompt entitled "Cards" for the assessor, we listed every card game we could think of. This helps prompt the person's memory regarding past activities enjoyed. It also gives the opportunity to ask the person if they might like to try to learn something new, i.e. a card game they have never played.
Included in this assessment are also common culture change practices around getting to really know a person including things like the person's typical routines, daily pleasures, and what brings meaning and purpose into their lives. Prompts are included to ask about loneliness, boredom and helplessness, the "three plagues" of institutionalization identified by the Eden Alternative.
In order to help us on our journey away from institutional care and towards person-directed care, we've developed a list of Principles of Excellence to consider adopting, following, hanging up in your office for all staff to see. These challenge us to rise higher, treat getting to know someone with utmost honor, turn the assessment process into a more normal "chit chat" than filling out a form, and get to know someone "over coffee" rather than "over a form."
We now include an additional “crosswalk” tool that will bring the information in this book up to date. Whenever you see a CMS regulation tag number, just use the crosswalk to quickly locate the new tag number, should you need it.
Although Phase 1 of the new CMS Requirements of Participation came into effect Nov. 29, 2016, regulation tag numbers were not changed until Phase 2 Nov. 28, 2017. Thus, the crosswalk is entitled Phase 2 Tag Crosswalk Effective Nov. 28, 2017.
